Switching to InFocus Plan Management is easy! Check out the following three ways to the make the
switch today based on your current situation:

Option 1

New to
the NDIS
  1. Request Plan Management in your planning meeting
  2. Once you receive your first plan, call us on 1800 928 437, sign-up online or email us
  3. We will set up your account
Option 2

Currently
with another
provider
  1. Let your current Plan Management provider know that you wish to end your service agreement
  2. Call us on 1800 928 437, sign-up online or email us
  3. We will set up your account
Option 3

Currently
Agency or
self-managed
  1. If you have a while until the end of your plan period ask the NDIA for an early plan review
  2. Request to change the way that your plan is managed, or
  3. Wait until your scheduled plan review to request the change
  4. Once you have Plan Management included in your plan call us on 1800 928 437, sign-up online or email us
  5. We will set up your account
